Fix instances accumulating in DB while their attachments were not stored because of MaximumStorageSize limit reached with a single patient in DB.

Introduction
============

This sample plugin enables Orthanc to serve DICOM modality worklists
that are read from some folder.

The shared library containing the plugin is created as part of the
build process of Orthanc.
