Thanks for your analysis! Regarding the two parts of your report:

1. **Wildcard search using GIN trigrams** was implemented during the refactoring of the PostgreSQL plugin: https://hg.orthanc-server.com/orthanc-databases/file/d17b2631bb6771dce0d71034bcaafefb8884bd87/PostgreSQL/Plugins/PostgreSQLIndex.cpp#L125
2. **Time period search** is solved through the fix of issue #41.

Both fixes will be part of forthcoming 2.2 release of the PostgreSQL plugins that is planned for July 2018.
