As I've previously reported on orthanc-users patient list in orthanc explorer may become very large to be processed or transfered every time - in my case it's already 1123 KB and it took 2182 ms to generate it on the server (87 ms for transfer in LAN) - in extreme cases of VPN it may lead to timeouts.
Maybe we should get the list after typing the first letter? 
I'm using postgres database backend, and I've got 732 GB of data.

It is otherwise quite efficient (after migrating to postgres, on sqlite it became painful to use around 500 GB)

Well, maybe I haven't read it carefully enough ; ) 
Anyway, we do not use the explorer for end-user, although it is otherwise surprisingly sufficient.
